Job Specification Job Summary Provides clinical nutritional and dietetic service to patients within the hospital. Common Responsibilities Ensures interdisciplinary input to attain full potential teamwork. Participates in educational activities for families and caregivers. Provides role modeling and mentoring with respect to clinical practice, continuing education, and career advancement/development. Participate in community nutritional activities. Participate in hospital /departmental/Program committees. Maintain professional growth and development through different in – service training activities to keep pace with technological developments and new trends in nutrition therapy. Cover the Dietician weekend duty or On-Call as required. Perform other duties as requested Specific Responsibilities. Performs nutrition assessment of referred patients to determine their nutrient needs and develop appropriate care plan which includes and not limited to: - Macro and micronutrient requirements and goals based on current recommendations. -Disease specific feeding choices. - Choices for delivery of nutrition e.g., intravenous, enterostomy. Monitors the implementation of nutrition plans and assesses patient progress to adjust therapy plans and attain optimum nutrition. Coordinates, plans and conducts programs to educate patients about nutrition issues, diet and food selections. Documents, records and processes information to assess therapy outcomes. Participate in multidisciplinary rounds and calculate the nutrient requirements of patients on therapeutic/enteral feedings as required. Calculates nutrient requirements of patients on therapeutic/enteral feedings as needed. Arranges or direct Assistant Clinical Dietitians to arrange discharge needs of patients to ensure continuity of care plan. Evaluates current practice using evidence-based outcome indicators where possible to ensure the therapeutic, patient and family needs are being met. Give consultation and Guidance for Assistant clinical dietitians.
