Job Specification Job Summary Assists in the process which ensures that patient care is delivered in accordance with established corporate wide policies and procedures and Nursing standards, and the corporation’s code of ethics. Provides education to staff based on theoretical and clinical expertise. This position will be accountable to ensure clinical resource and continuity of patient care during her/his duty hours. Common Responsibilities Responsible to a Charge Nurse and Head Nurse to provide professional, quality patient care. To fulfill the responsibilities above, in collaboration with nursing personnel and members of other disciplines, engages in the following activities: Provides direct patient care. Provides guidance for and supervision of personnel accountable to you. Coordinates nursing services with the services of other health care disciplines. Assumes responsibility for staffing and scheduling personnel. Assignments reflect appropriate utilization of personnel. Fosters a climate conducive to education experiences for nurses and students. Evaluates the quality and appropriateness of care. Accepts organizational accountability for services provided to recipients. Specific Responsibilities: Provides direct patient care and serves as clinical resource, role model/mentor in the delivery of patient care. 1 Maintains and demonstrates clinical competence in all aspects of care provided to patients on specific unit. 2 Acts as clinical resource in assessing complex patient care needs/situations and in determining appropriate interventions; recommends alternative approaches. 3 Utilizes critical thinking when making clinical judgments, managing conflicts and solving patient care problems/issues. 4 Assesses and reassesses patient needs and response to care, treatment and education, as per Policy 5 Develops, implements, evaluates, revises and provides input to the patients’ plan of care and provides care according to the plan of care. 6 Demonstrates effective use of organization, management and nursing theories in the delivery of patient care. 7 Anticipates, recognizes and manages emergent and unsafe situations/practices and initiates appropriate and timely interventions. 8 Demonstrates knowledge of legal aspects of patient care and nursing practice. 9 Acquires knowledge and skills on current clinical and patient care management practices and applies these in the provision of patient care. 10 Accepts accountability for care provided to patients. 11 Actively communicates the care of the patient thru communication and written documentation to all disciplines involved in the patients care. 12 Supervises ancillary staff involved in the patient’s plan of care. Assists in management of the department. 1 Assists and supports unit staff in developing and maintaining clinical competence. 2 Coordinates/collaborates with providers of patient care within the delivery system for the effective and timely delivery of care. 3 Provides appropriate and timely feedback on the quality of care provided by the unit staff. 4 Displays a spirit of teamwork and maintains a collegial relationship with unit staff and other healthcare providers. 5 Participates in unit meetings and contributes in achieving objectives; reads communications and acts upon it positively. Participates in the Performance Improvement process aimed to improve patient outcomes. 1 Complies with corporate policies/procedures and nursing department policies/procedures and standards. 2 Recommends changes and supports processes that allow for the development of alternative plans to achieve desired client-centered and cost effective outcomes. 3 Assists in the development and/or implementation of corporation-wide and department-based policies/procedures related to assessment and care of patients, continuum of care and patient / family education. 4 Assists/participates in problem identification, data collection and development of action plans to improve performance. 5 Assists/participates in the implementation of ongoing corporate, department and unit performance improvement initiatives. 6 Coordinates the practice of all healthcare professionals and ensures that they function within the scope and standards of their professional practice.*Participates in multidisciplinary practice and ensure a culture of teamwork. 7. Enhances own professional growth by meeting mandatory educational requirements as stated in the policy for continuing and in-service education. 8 Adapts positively to corporate changes in practice and delivery of patient care. 9 Accepts that compliance with corporate standards is a condition of employment. Maintains and promotes customer satisfaction through effective customer service. 1 Acquaints/guides staff on customer service principles and practices. 2 Is conversant with patient’s rights and her/his responsibilities in meeting them 3 Maintains confidentiality in relation to patient, employee, and corporate information. 4 Practices professional communication and customer service skills with all customer groups; acts in a courteous, sensitive and professional manner. 5 Meets customer needs without delay i.e., patient, visitors, relatives and other members of staff 6 Ensures that patients/family/significant others are provided with adequate and appropriate information and reassurance prior to any medical and nursing intervention. 7 Implements measures to modify pain and promote comfort with minimum delay. 8 Conducts self that gives credit to the nursing profession and a good public image to the institution. 9 Provides care and services according to the scope and standards of nursing practice and adherence to the Code of Conduct / Ethics for nurses. 10 Respects patients’ rights and privacy, which is compliant to the Privacy Policy. Administers medications and therapies in a safe, timely and effective manner. 1 Administers medication by application of the seven rights. 2 Administers medication only if well versed with the drug’s indications / contraindications, effects/side effects, relevant warnings and precautions. 3 Utilizes proper, safe and aseptic technique in the administration of intravenous, intramuscular, subcutaneous and intradermal injections and other invasive procedures related to drug therapy. 4 Adheres to prescribed therapy; seeks clarification of orders that are incomplete, questionable or ambiguous. 5 Documents and informs concerned staff of patient’s response to drug therapy. 6 Adheres to policies related to medication administration. Clinical documentation reflects application of the nursing process. 1 Documents accurate and current patient assessment. 2 Updates nursing plan of care indicating current patient problems/needs and relevant interventions. 3 Documentation of care provided reflects the steps of nursing process using the recommended documentation format. 4 Documentation shows evidence of discharge planning, patient/family teaching and response. 5 Documents patient care provided in accordance with nursing standards and policies on documentation. 6 Data is legibly and correctly entered and signed on all relevant documents. Provides care that is relevant to each age group. 1 Demonstrates basic knowledge of normal growth and development of all age groups (neonate, infant, toddler, pre-school child, school child, adolescent, young, middle and older adults). 2 Modifies data collection methods to adequately assess the holistic needs and treatment responses of a specific age group. 3 Determines nursing interventions that will meet the developmental needs of a specific age group. Provides patient/significant other with self-care and health maintenance knowledge base and skill. 1 Demonstrates knowledge of the basic principles of patient education. 2 Identifies patient education needs at admission and during hospitalization / care in residential or domiciliary setting. 3 Plans teaching/learning activities with the patient/family/significant others. 4 Teaches the patient the basic principles of health maintenance and self-care. 5 Provides patient with the basic information about his/her disease and of the limitations and expected outcomes of treatment. 6 Provides patient with knowledge related to prescribed medications (drug name, effects/side effects, time, route, duration of therapy, and signs/symptoms to report). 7 Teaches and assists patient/family/significant others in acquiring the necessary skills required in meeting patient care needs. Adheres to infection control and environmental care standards and practices. 1 Adheres to standard precautions and isolation procedures at all times, as applicable. 2 Uses appropriate personal protective equipment (PPE) according to Infection Control policy. 3 Collects and transports specimen in accordance to policy 4 Disposes of healthcare wastes in accordance to waste management requirements. 5 Demonstrates knowledge of and complies with safety policies and procedures, the use of MSDS sheets, hazardous procedures and safety codes. 6 Demonstrates knowledge of and complies with security issues i.e., personal protection, protection of personal and property. Attends to emergency / crisis situations calmly and effectively. 1 Is aware of emergency cart status and location (if applicable). 2 Adheres to the policy for checking and replenishing emergency supplies, as applicable. 3 Identifies and locates emergency drugs; is knowledgeable of their use and side effects. 4 Identifies emergency equipment and demonstrates its proper use/application. 5 Identifies cardiopulmonary arrest and performs cardiopulmonary resuscitation (CPR) correctly and without delay. 6 Complies with the Code Blue policy and documenting on the Cardiopulmonary Resuscitation Flow sheet. 7 Is aware of the Disaster Plan and his/her role and responsibilities. General Responsibilities: Reports for duty according to roster and work requirements. 1 Adheres to department policies on attendance and tardiness. 2 Complies with the dress code designated for area of assignment. 3 Leaves for and returns from meal break according to schedule. 4 Accepts changes in work schedule to meet staffing needs of the unit/department. 5 Ensures that the department head approves the overtime prior to assuming work. 6 Obtains approval of immediate supervisor before scheduling time off. Demonstrates a working knowledge of resource consumption. 1 Considers staff capability and patient acuity when making patient assignment. 2 Accurately uses staffing matrix and adjusts staffing mix to meet current needs for patient care 3 Demonstrates appropriate use of supplies and equipment. 4 Demonstrates compliance to administrative policies related to staff attendance. Promotes the development of clinical skills of the Staff Nurse orientee, nurse trainee and nursing students. 1 Acts as a preceptor to new nurse employees, nurse interns and nursing students during orientation/internship period. 2 Provides professional guidance to staff through example and education. 3 Works closely with Staff Nurse orientee, Nurse trainee and Nursing students to assess level of functioning, competence, and customer service skills. 4 Provides/facilitates education and training opportunities to meet educational needs of orientees, interns and students. 5 Fosters an environment that is conducive to learning and attainment of competence. 6 Participates in the performance evaluation of nurse orientees, interns and students; completes evaluation in a timely
